Registration Information 2012-2013

Registration for the 2012-2013 school year is as follows:

Registration for the 2012-2013 school year is as follows:

 

**NOTE : Idaho Code Section 39-4801, Idaho Department of Health and Welfare Rules, IDAPA 16, Title 02, Chapter 15 "Immunization Requirements for Idaho School Children." All children in pre-school or entering into grades K-12 must meet immunization requirements at registration and before attendance. Registration is from 8:00 a.m. - 4:00 p.m.

All Buildings closed from 12:00 to 1:00 for lunch.

 

· Elementary Schools - All elementary schools  

~   August 06 - Register both new and returning K-4 graders

 

· Hacker Middle School (HMS)

~   August 06 - Register both new and returning 5-6 graders

 

· Mountain Home Junior High School (MHJH)

~   August 03 -

§  NEW 7-8 grade students, please call 587-2590 to make an appointment to register at the junior high

~   August 09 -  

§  Returning 7-8 grade students need to register in person at the junior high

 

· Mountain Home High Senior School (MHHS)  

~   August 01 -

§  NEW 9-12 grade students, please call 587-2570 to make an appointment to register at the high school

~   August 06-13 -

§  Returning 9-12 grade students may register online

~   August 09-10 -

§  Returning 9-12 grade students who didn't register online may register in person

~   August 13 -

§ Starting August 13, all registrations must be completed in person

 


MOUNTAIN HOME SCHOOL DISTRICT REQUIRES THAT ALL STUDENTS AND PARENTS/GUARDIANS SIGN A NETWORK USE PROCEDURE - AGREEMENT BEFORE THE STUDENT IS ALLOWED TO USE A COMPUTER AND/OR INTERNET.

THIS SIGNED AGREEMENT IS KEPT ON FILE AND CARRIES OVER FROM YEAR TO YEAR FOR GRADES 5-12.  IF A PARENT/GUARDIAN WISHES TO MAKE A PERMISSION CHANGE, IT IS THE RESPONSIBILITY OF THE PARENT/GURADIAN TO CONTACT THE SCHOOL.  AGREEMENTS FOR GRADES K-4 MUST BE SIGNED EACH SCHOOL YEAR.

   
What is Needed to Enroll

 

Who must attend school in Idaho?  (Idaho Code 33-202) Idaho law requires that all resident children between the ages of 7 and 16 must attend school.  In this district, a resident child refers to a child whose parents' or legal guardians' home is located within the boundaries of the Mountain Home School District. Does my child have to have all of his/her Immunizations before I can register him/her for school?  (Idaho Code 39-4801) Idaho Code Section 39-4801, Idaho Department of Health and Welfare Rules, IDAPA 16, Title 02, Chapter 15 "Immunization Requirements for Idaho School Children." Exemptions to Immunization Requirements:  Idaho law allows a parent/guardian to claim an exemption from immunization requirements for their child for medical, religious, or personal reasons. All exemptions must be documented on the official Idaho Exemption Form provided by the Idaho Department of Health and Welfare, Immunization Program (available at www.immunizeidahoschools.com). A medical exemption requires the signature of a licensed physician and certification that the child has a medical condition, which prevents him or her from receiving one or more of the required vaccinations. The law does not allow parents/guardians to claim an exemption because the shot record is lost or incomplete, or because it is too much trouble to contact the physician or clinic and obtain a copy.
